Under-eye filler is a hyaluronic acid gel injected into the tear trough — the hollow that runs from the inner corner of the eye down onto the cheek. The treatment aims to raise this depression, softening the shadow that can create a tired appearance. How much volume can be added and where it is placed depends on the anatomy of the orbit: the thickness of the skin, the support from the cheekbone, and how the fat pads below the eye have changed over time.

The cause of a dark circle is not always the same, and filler is not always the answer. Where the issue is a shadow cast by a hollow, volume can help. Where it is pigmentation in the skin itself, or the colour of muscle seen through very thin skin, filler will not change it. The assessment is about the entire mid-face. The plan is made against the projection of the cheekbone and the transition from the lower eyelid to the cheek, not just the trough itself.

The area under the eye does not exist in isolation. Its appearance is directly related to the volume and support of the cheek below it. In many cases, the first step is not to fill the tear trough itself, but to restore structure to the cheek first. Adding volume directly into the trough without this underlying support can lead to puffiness or an unnatural result. The goal is a smooth, uninterrupted contour from the lower lid down to the cheek, and this often begins away from the problem area.

Is under-eye filler the same as surgery for eye bags?

No, they address different anatomical issues. Filler is an injectable gel that adds volume to fill the hollow, or tear trough, that creates a shadow. Lower eyelid surgery, or blepharoplasty, is an operation that repositions or removes the fat pads that bulge forward, and sometimes removes excess skin. One treats a lack of volume; the other treats a surplus of tissue. The correct approach depends on the underlying cause of the under-eye appearance.

Is under-eye filler permanent?

The body has natural enzymes that break down hyaluronic acid, so the gel is gradually absorbed over time. The effect of the filler diminishes as this happens. The rate at which the body metabolises the product is individual and depends on multiple factors. The treatment can be repeated when the original appearance returns, but the substance itself does not remain in the tissue indefinitely.

What is the gel used in under-eye filler made of?

The fillers used are gels made from hyaluronic acid, a complex sugar molecule that occurs naturally in human tissues, especially the skin and connective tissue, where it helps retain moisture. The hyaluronic acid in fillers is synthesised in a lab and cross-linked to form a stable gel. Different formulations exist, chosen based on the specific area being treated, such as the tear trough or the supporting cheek.

Can under-eye filler be reversed?

Fillers made from hyaluronic acid can be dissolved. This is done by injecting hyaluronidase, an enzyme that specifically breaks down hyaluronic acid. The enzyme accelerates the natural process of absorption by the body. This property is specific to hyaluronic acid-based products and is a consideration in their use for facial injections. The decision to dissolve filler is a clinical one, made with the treating doctor.

Where is under-eye filler treatment performed?

The treatment is performed by a doctor in a clinical procedure room within an accredited Istanbul hospital. While it is not a surgery requiring general anaesthesia, it is conducted in a formal medical setting.
