Canthoplasty is an operation on the lateral canthal tendon, the structure that anchors the outer corner of the eyelids to the bone. The procedure aims to reposition this point, changing the horizontal length and upward tilt of the eye. How much the shape can be altered depends on the anatomy of the eye socket, the existing laxity of the lower eyelid, and the relationship between the eyeball and the orbital rim. These are fixed points around which any change must be planned.

The same desired shape requires a different surgical approach on every face. An eye's appearance is defined by its relationship to the brow, the cheekbone, and the nose. A canthoplasty is therefore not planned to match a reference photograph, but to create a harmony with the existing facial structure. The final angle of the eye must suit the curve of the orbit and the projection of the surrounding bones, or the result will look displaced rather than natural to the face.

Where are the incisions for a canthoplasty?
