Blepharoplasty is an operation on the thin skin of the eyelids, the muscle underneath, and the pockets of fat that sit behind them. On the upper eyelid, it can address skin that hoods over the lash line. On the lower eyelid, it can address puffiness or bags. The limits of what can be changed depend on your skin's elasticity, the position of your eyebrows, and the shape of the bone around the eye socket. These factors determine how much tissue can be safely removed.
A particular request, such as for a more open eye, will be planned differently for each person. The surgery is not considered in isolation. The surgeon assesses the relationship between the eyelid, the position of the brow, and the transition to the cheek. Simply removing tissue without considering these surrounding structures can lead to an unnatural or hollowed appearance. The plan is therefore made against the patient’s own anatomy, not a reference photograph, aiming for a result that belongs to that specific face.

For the upper eyelid, the surgeon will assess whether excess skin is interfering with your field of vision, which makes it a functional issue as well as an aesthetic one. For the lower eyelid, the focus is often on the transition to the cheek. Puffiness can be caused by fat moving forward, but a hollowed tear trough below it is often about volume loss in the mid-face. Addressing the eyelid alone might not be the complete answer if the problem extends onto the cheek.

Where are the incisions for upper and lower eyelid surgery?
For an upper blepharoplasty, the incision is placed within the natural crease of the eyelid, making it very inconspicuous when the eye is open. For a lower blepharoplasty, the incision can be made either just below the lash line on the outside, or on the moist inner surface of the eyelid where it is not visible at all. The approach is selected based on the specific anatomy and whether excess skin needs to be addressed in addition to the fat pads.
What is the difference between a brow lift and an upper blepharoplasty?
An upper blepharoplasty addresses the eyelid itself, by removing excess skin or adjusting fat that sits on the lid. A brow lift addresses the position of the forehead and eyebrows. Sometimes, what appears to be heavy upper eyelid skin is actually caused by a low or descended brow. The consultation assesses which structure is the primary cause of the concern, as operating on the eyelid alone will not correct a low brow.
Does lower blepharoplasty get rid of dark circles?
Lower eyelid surgery is designed to correct puffiness or bags, which are caused by protruding fat pads. It can improve the appearance of a shadow cast by these bags. However, it does not change skin pigmentation, which can be one cause of dark circles. If the circle is a hollow groove (the tear trough), that may relate to volume loss in the upper cheek, which is a separate anatomical consideration.
Can upper and lower eyelid surgery be done at the same time?
Yes, it is common to perform surgery on both the upper and lower eyelids during a single operation. This is sometimes referred to as a quadrilateral blepharoplasty. The decision depends on the surgeon's assessment of all four eyelids and the surrounding facial structures. The aim is to create a balanced relationship between the upper and lower lids that is appropriate for the individual's face.
Where is the blepharoplasty operation performed and who will be there?
The surgery is performed in a fully accredited hospital in Istanbul, which will be named in your written plan before you travel. In the operating room with you will be your surgeon, the clinic's own anaesthesia team, and the hospital's surgical nursing staff. A dedicated interpreter from the clinic is also present throughout your time at the hospital to ensure clear communication regarding your procedure and your eye care.
What should I bring to my blepharoplasty consultation?
Please prepare a list of all your medications, vitamins, and herbal supplements. Be ready to discuss your full medical history, especially any specific eye conditions such as dry eye, glaucoma, or allergies, and any previous eye surgery. If you wear glasses or contact lenses, bring them. It can also be helpful to bring a few photographs of your face from several years ago to help illustrate the changes you have noticed.
