A lip lift structurally alters the area between your nose and your upper lip, known as the philtrum. During the surgery, the surgeon makes an incision directly beneath the base of your nose. A specific amount of skin is excised, and the remaining tissue is lifted and sutured into place. This elevation pulls the upper lip upward, exposing more of the pink lip tissue (vermilion) and altering the resting position of your lip.
The surgical plan is determined by your specific facial anatomy, particularly the length of your philtrum and your dental structure. Your upper jaw projection and tooth visibility dictate how much skin can be safely removed. If your philtrum is already short, removing more skin might expose your teeth excessively or prevent your lips from closing naturally. The mechanical limit of the lift depends entirely on your existing skeletal and tissue framework.

A lip lift is distinctly different from lip fillers. While fillers inject volume to inflate the lip, a lip lift surgically shortens the skin above it — a change to the structure rather than to the volume. It does not add physical volume to the tissue itself. The most common technique, the bullhorn lip lift, places the incision strictly along the natural contours of the nostril base, separating it from procedures that cut directly along the lip border.
Before your consultation, compile a complete history of any previous lip modifications. You must disclose if you have existing dermal fillers in your lips, as this alters the baseline tissue and affects surgical planning. Bring records of any past dental surgeries or orthodontic work, as your tooth alignment impacts the final structural outcome. Be prepared to point out exactly how much tooth visibility you currently have when your mouth is relaxed.

What is the difference between a lip lift and lip fillers?
A lip lift is a surgical procedure that removes skin to raise the upper lip, exposing more of the pink tissue without adding volume. Lip fillers use injectable gels to temporarily inflate the lips, adding physical mass. A lift changes the structural position, while fillers alter the volume capacity of the tissue.
Does a lip lift leave a visible scar?
A lip lift requires an incision, which means a scar is formed. In a bullhorn lip lift, this incision is strategically placed along the natural creases where the base of the nose meets the upper lip. While the placement is chosen to camouflage the incision line, the way the scar heals depends entirely on your cellular biology.
Will a lip lift show my teeth?
Yes, structurally elevating the upper lip often increases dental show. By shortening the philtrum, the resting position of the lip is raised, which typically exposes more of your upper teeth when your mouth is slightly open. Your surgeon will measure your current tooth visibility to determine how much skin can be safely removed.
Can I get a lip lift if I have fillers?
Having existing fillers affects the baseline measurements of your lip tissue. Most surgeons require that any hyaluronic acid lip fillers be completely dissolved before performing a lip lift. This is so the surgical plan is based on your own anatomy rather than on tissue that is still inflated and will not stay that way.
Does a lip lift change the shape of the nose?
Because the incision for a lip lift is located at the base of the nose, there is a risk of slightly altering the nostril shape if not performed carefully. The skin is anchored to the tissues beneath the nose, which can cause mechanical tension. The technique used aims to isolate the lip tissue to prevent widening the nostril base.
Is a lip lift permanent?
The strip of skin removed from the philtrum does not grow back, so it is not a change that reverses on its own. That is not the same as a result that never changes: your face goes on ageing, your skin elasticity goes on changing, and the tissue around the lip changes with it.
