The surgeon determines the tissue's composition during a physical examination. Glandular tissue feels firmer and is typically located directly behind the areola, while fatty tissue is softer and more diffuse. The feel and location of the tissue on examination is the primary guide. In some cases, imaging such as an ultrasound may be used to provide more detail, but the clinical assessment is the most important part of planning the correct surgical approach for the chest.

The location and extent of incisions depend entirely on the surgical plan. For tissue removal through excision, the incision is most often placed along the lower edge of the areola. Where liposuction alone is sufficient, small entry points are made in less conspicuous areas. If a significant amount of skin must be removed, the incisions will be more extensive. The goal is always to place incisions where they are least visible within the final contour of the chest.

The glandular and fat cells removed during the operation do not grow back. However, the final contour can be affected by future life events. Significant weight gain can cause remaining fat cells in the chest and surrounding areas to enlarge, altering the shape. Hormonal imbalances or the use of certain medications or substances can also stimulate the growth of any residual glandular tissue. Maintaining a stable weight is a key component in the longevity of the surgical outcome.

Asymmetry is common. It is very frequent for the amount of glandular or fatty tissue to be different on each side of the chest. The surgical plan is not a single action applied to both sides; it is tailored individually to each pectoral area. The surgeon assesses the volume and composition of tissue to be removed from the left and right sides separately, with the aim of creating a more symmetrical and balanced chest contour relative to the patient's overall torso.

Bring a list of all medications, supplements, or hormonal products you have taken, both now and in the past. It is also helpful to have photographs of your chest from previous years, especially if your weight has changed significantly. This helps the surgeon understand the history of the condition. Most important is to be ready to describe what you see and what you hope to address, in your own words. No medical terminology is needed.
