A midface lift addresses the anatomical changes in the central third of your face. During the surgery, the surgeon accesses the underlying muscle and fat pads, usually through incisions hidden in the hairline or inside the mouth. These tissues are elevated and secured to higher anchor points on your facial bones. The procedure focuses specifically on the area between your lower eyelids and your mouth, aiming to restore the structural position of the cheek volume.
The anatomical outcome depends heavily on your existing bone structure and skin elasticity. Patients with strong cheekbones provide better anchor points for tissue elevation. If your skin lacks sufficient elasticity, lifting the underlying fat pads may require additional skin removal to prevent gathering. Your unique facial skeletal structure dictates how far the tissues can be mechanically repositioned without creating unnatural tension.

A midface lift is frequently confused with a traditional full facelift. While a full facelift targets the jawline, jowls, and neck, a midface lift isolates the cheek and infraorbital (under-eye) regions. The structural anchor points differ; midface procedures often utilize absorbable devices or deep sutures to suspend the tissue vertically. It does not address loose skin in the lower face or neck area.
When attending your consultation, bring photographs showing your facial structure from previous years to help identify your baseline anatomy. You should clearly indicate the specific areas of your cheeks or under-eyes that concern you. Be prepared to discuss any past facial procedures, including dermal fillers or thread lifts, as previous internal scar tissue can affect surgical access and tissue mobility.
