Do Breast Implants Get Bigger or Smaller After Surgery?

They appear smaller as swelling goes down.

This is probably the most confusing and emotionally exhausting part of the recovery process: you wake up from the surgery, still a little dazed but happy, and the first thing you do is look at your chest and decide if breast implants are too big, too small, or just right. To make the situation even more complicated, you may come across completely opposite stories on internet forums. Some women say that once the swelling went down, their implants “shrunk” drastically, while others are sure that their breasts “fluffed up” and even got bigger and rounder a few months later. Such a paradox makes many patients so worried that they even start to doubt if they have chosen the right size.

The question: “Do they get bigger or smaller?” has the surprising answer of both but at different stages of the healing process. The path from the operation table to the final “forever” result is going to be a biological mixture of things like fluid retention, muscles getting tight, skin stretching, and gravity. At Lin Europe Clinic, we make sure that you are mentally prepared for this roller coaster of changes, so you won’t panic when changes occur and know that your true size is generally revealed between 3 and 6 months after surgery.

Phase of “Shrinking”: 1 to 6 Weeks

Right after the operation, your breasts will probably be at their absolute largest, but their shape will be at its worst. This is because of post-surgical swelling and an accumulation of fluid. In simple terms, your body will send a lot of lymphatic fluid and blood to the area in order to heal the tissue. So, your breasts will look swollen, shiny especially high on the chest, and the redness may run all the way up to the collarbone.

While this fluid is slowly absorbed, you will see a decrease in the total volume you had before. This decrease can sometimes be so shocking that it makes people panic and say, “My implants are shrinking!” However, implants do not shrink in volume. The silicone volume is constant. What you are currently experiencing is the disappearance of inflammation and water retention. Usually, patients go down a cup size or two during this time. Also, the fact that you for sure wear a medical-grade compression bra that is designed to flatten the tissue to prevent fluid buildup, contributes to the feeling of “flatness” or smallness compared to your initial post-op reveal.

The “Growing” Phase: Drop and Fluff

You are just about to think they might get too small when the second and the most spectacular phase begins: the well-known “Drop and Fluff. “ After the Dual Plane-type implant (the one placed under the muscle), the major pec muscle will be kind of paralyzed and also in spasms. It works as if it were a tight rubber band that presses the implant to the ribs almost as if it were flattening it while also holding it high. Because of such a strong compression, the real projection of the implant is not visible; it is smaller and flatter than what it actually is.

It is from months 3 to 6 that the muscle starts to get used to the implant and gradually loses its tension. When the muscle finally releases, it is no longer pressing the implant against the ribs, so the implant can both settle downwards in the lower breast area and project forwards from the chest. This causes the lower pole of the breast to fill out and become rounded significantly. Even though the volume in cc’s has not changed, the visual surface area has increased remarkably. The breasts look more voluminous, softer, and more “bigger” simply because they are projecting forward instead of being flattened against the ribs.

The Texture Factor: Why Timing Varies

It should not be overlooked that the rate at which these changes take place depends on the surface of your implant.

  • Smooth Implants: Due to their slippery surface, they usually manage to slide into the lower pocket and “fluff” sooner, very often in less than 3 months.
  • Textured Implants: These types are meant to grip the tissues like Velcro. As a result, it might take them a bit longer—sometimes from 6 to 9 months—before they can fully “drop” and be “settled” in the final position because tissue adhesion holds them at the top for longer. Knowing what kind you have is very helpful in terms of patience; if your implants are textured, relax if they still look smaller or higher at the 4th month.

The Psychological Factor: “Boob Greed”

Boob Greed” or size dysmorphia is another psychological phenomenon that is quite well known. Such a change as going from a bright A-cup to a full D overnight is very shocking. Your brain initially even thought the new breasts to be “massive” for a few weeks.

Humans, however, can very quickly get used to things. After just a few months, your brain will come up with a new “normal.” What was massive on the first day now looks normal in the mirror. Many women mistakenly believe that this habituation to the new body has made their implants shrink when it’s actually their body image that has changed and they have just mentally adapted. This is the main reason why we recommend patients to stay at least a year before thinking about going through another surgery if they feel the size isn’t right. Your perception of size changes usually goes way beyond what the implants do.

Expectations in Turkey

We in Lin Europe Clinic in Turkey stress the importance of keeping patients calm during their follow-up consultations. Besides just cup size, we also assess the chest in terms of the tissue quality and relaxation. We often tell patients, “Don’t get married to your result until month six.”

Also, during the “Shrinking” or “Fluffing” phases, we discourage patients from splurging on costly bras. A week 4 perfect-fitting bra will probably have empty cups by week 8 (due to the loss of swelling), and then it will become too tight in the bottom by month 4 (because of the drop and fluff). If you trust the process, you’ll neither waste your money nor your emotional energy on temporary ‍‌‍‍‌‍‌‍‍‌changes.

Frequently Asked Questions About Size Changes

What‍‌‍‍‌‍‌‍‍‌ is the “Drop and Fluff” technique?

It is a surgical technique. However, it is a phase of natural healing during which a very tight pectoral muscle relaxes, thus, the implant can settle lower and the bottom of the breast can expand and become more rounded.

Why do my implants look smaller in the morning?

This happens most of the time due to a fluid distribution; the swelling is usually less overnight when you are lying flat and it might increase slightly during the day as gravity pulls the fluid down.

Do breast implants shrink over years?

Silicone implants do not shrink or lose volume over time. Nevertheless, if you lose a lot of weight, the fatty layer of your natural tissues that covers the implant may get thinner thus, the size of the whole breast may diminish.

Does the “high and tight” look mean they are too big?

No, the “high and tight” look results from muscle spasms and swelling that force the implant upward. When the muscle relaxes, the implant will fall into a natural teardrop shape, which is less bulky and more appealing.

What is “Boob Greed”?

This is a very typical psychological behavior where a patient gets accustomed to their new breast size very quickly and starts feeling that they are “too small.” Most of the time, they regret not getting bigger implants even though, at first, they were satisfied with their ‍‌‍‍‌‍‌‍‍‌size.
