When you get breast augmentation, it is only natural that you plan on enjoying the benefits for years to come. You pick a size, you fall in love with the new figure, and you think that the 350cc of today will be the same in 10 years. However, a very frequent worry of those who have had implants for a long time is that their breasts are getting smaller day by day in spite of the implants. They see themselves in the mirror 5 years after the operation and ask, “Did my implants shrink or have I just imagined it?”

To answer the question, “Do breast implants shrink?“, one has first to consider what they’re filled with. Silicone implants cannot shrink; they are chemically stable solid entities. Saline implants, on the other hand, can lose volume if tiny leaks develop in the shell. Yet even if your implant is flawless, your breasts can be different. Silicone vs. Saline: The Material Matter

If you want to know whether your implants can shrink, at first you need to figure out what they are made of.

  • Silicone Gel Implants: These are really the new standard in beauty enhancement. They consist of a very cohesive, semi-solid gel (which is often referred to as “gummy bear” gel) inside the implants. Due to the fact that the filler is a solid, it cannot evaporate or leak through the shell intact. If you’ve got silicone breast implants, they will nigligibly change in volume over your lifetime. Even if the outer shell breaks, the gel usually tends to stay in the implant rather than getting lost.
  • Saline Implants: These implants are comprised of silicone shells that have been filled with sterile saltwater. Over a long period, it is pretty much inevitable for the valve to deteriorate or for the shell to develop some tiny “fold flaws”. As a result, the implant will very slowly leak—maybe just a few milliliters a month— hence it becomes deflated and shows some wrinkling and loss of shape. If you notice that your one breast suddenly became smaller than the other in a matter of weeks, most probably this is the result of the saline deflation.

The Illusion of Shrinking: Weight Loss and Atrophy

For many ladies who have silicone implants, if they finally feel that their breasts have reduced in size, their implant is not the cause – it is their own body tissue. You breast is a multi-layered structure: it is your implant combined with your natural glandular tissue and the layer of fat on top.

  • Weight Loss: Any time you lose weight, you lose fat everywhere, including the breasts. The implant remains unchanged but because the fat covering it becomes thinner, the overall breast circumference is reduced.
  • Post-Pregnancy Atrophy: After breastfeeding, the glandular tissue normally involutes (shrinks). So, you may have had 200cc of natural tissue over your 300cc implant before. If this natural tissue shrinks to 50cc, your total breast volume drops although the implant is perfect. Patients usually perceive this “empty” sensation as the implant shrinking, whereas, in fact, they have only lost their natural cover.

Capsular Contracture: The Squeeze Effect

There is one condition that can actually make the implant smaller by squeezing it inside: Capsular Contracture.

The body naturally creates a layer of scar tissue to enclose the implant. For most women, this capsule remains soft and spacious. On the other hand, in rare cases, the scar tissue may become thicker and tighter as time goes by.

Imagine​‍​‌‍​‍‌​‍​‌‍​‍‌ you’re holding a balloon in your hand. The balloon is not deflating, but it appears smaller because it is being squeezed into a tight, hard ball. So if your breasts are firm, higher than usual, and smaller looking, you might be experiencing capsular ​‍​‌‍​‍‌​‍​‌‍​‍‌contracture. The implant was not reduced; it is just that the scar tissue has got very tight around the implant.

“Boob Greed” and Adaptation

We must not forget about the psychological aspect. We label the term “Boob Greed” but the clinical term is hedonic adaptation.

When you start wearing implants they feel heavy and look really large since you are comparing the new size to your previous flat chest. Your brain processes them as “terrific.”

After a year or two, your brain undergoes changes. Your perception adjusts. You no longer see “extra volume” but just “my body.” This process of getting used to frequently misleads women into thinking that their breasts have actually gone through some physical shrinking. Actually, they haven’t; you have simply become so accustomed to them. Do silicone implants evaporate?

Silicone is a non-volatile solid gel and does not evaporate or leak through the shell; hence the volume remains constant for a very long time unless the shell physically ruptures and the gel leaks.

Why does one breast look smaller years later?

Asymmetry that occurs later is mostly caused by uneven weight loss, hormonal changes impacting one breast gland more than the other, or a silent rupture (if using saline) resulting in one-sided deflation.

Does gravity make implants look smaller?
